Informed Consent and Liability Waiver
This waiver of liability includes any risk of attending consultations, engaging in Zoom sessions, coaching, classes, workshops or other services provided by Esther Seegers of Mind Yourself Ayurveda LLC.
Esther Seegers provides counselling regarding Ayurveda, Yoga Therapy and Jyotish. Esther is constantly engaged in continuing education in all the above areas.

In addition to preventative medicine, many of our patients come to Mind Yourself Ayurveda with a variety of health concerns ranging from autoimmune diseases to cancer. Clinical Ayurvedic practitioners are able to treat the full range of physical and emotional ailments while also working closely with your western healthcare team.

Ayurvedic Practitioners are not licensed physicians, and Ayurvedic services are not licensed by the state. Esther Seegers at Mind Yourself Ayurveda LLC provides wellness coaching and do not seek to represent or replace modern licensed health care professionals and therapies. Services provided are designed to be an ancillary health aid and are not suitable for primary medical diagnosis and treatment and are not intended as a replacement for licensed medical therapies or clinical mental health care. It is your responsibility to maintain a relationship with your medical doctor.

By checking the Informed Consent check box, the person named in the intake and residing at the declared address understands that any Ayurvedic treatments or Yoga therapies, lifestyle adjustments or Jyotiá¹£h remedies received from Esther Seegers are designed to be an ancillary health aid and are not suitable for primary medical diagnosis or treatment and are not a replacement for licensed medical therapies or clinical mental health care. You acknowledge that you are seeking a consultation for educational purposes based on the principles of Ayurveda, Yoga, and Jyotish.

You acknowledge and agree that you are fully responsible for your own well-being during your consultation or therapy sessions, and subsequently, your choices and decisions.

You understand that all comments and ideas offered by Esther Seegers are solely for the purpose of aiding you in achieving your defined goals and you can disregard any comment or idea offered which is not in your best interest. You have the ability to give your informed consent, and hereby give such consent to assist in achieving such goals.
Confidentiality
All communications between you and your Therapist-Practitioner will be held in strict confidence unless you provide written permission to release information about your recommendations. If you participate in a couple’s consultation, Esther Seegers will not disclose confidential information unless all person(s) who participated with you provide their written authorization to release. In addition, Esther Seeegers will not disclose information communicated privately to him or her by one family member to any other family member without written permission.
As a practice we share pertinent history and recommendations with other health practitioners working with you when you have provided written permission. This will always be done in a confidential way respecting your integrity and privacy.
We use third party payment systems and do not collect or store any financial information.
All sessions are recorded and stored in a HIPAA compliant cloud server. You acknowledge that the use of technology cannot always be completely secure and accept the risks to confidentiality in the use of email, text, phone, Skype, and other technology.
Liability Waiver
You declare and represent that no promise, inducement or agreement not expressed in this agreement has been made. You hereby release, waive, and acquit your therapist/coach, any agents, successors, assigns, personal representatives, executors, heirs and employees from every claim, suit action, demand or right to compensation for damages claimed or that you may have arising out of your own acts or omissions or acts and omissions of your Therapist/Coach as a result of any advice given otherwise resulting from the therapeutic/coaching relationship contemplated by this agreement.
Moreover, in consideration of the benefits to be derived from the counseling, the receipt whereof is hereby acknowledged, you hereby indemnify and hold harmless, release, remise and forever discharge and covenant not to sue or hold legally liable Esther Seegers or the staff at Mind Yourself Ayurveda LLC from any and all claims, demands, damages, actions, or causes of action whatsoever related to the counseling process.

You have read the above release and waiver of liability and fully understand its contents. You are aware that this is a waiver and release of potential liability and a contract between you and Esther Seegers and the staff of Mind Yourself Ayurveda LLC. You voluntarily agree to the terms and conditions stated above.

Mind Yourself Ayurveda authorizes you to access the website and materials using your personal computer, and to save and/or print certain materials for the sole purpose of facilitating your own personal, non-commercial use of the information. You may not sell, rent, lease, loan, sublicense, modify, create derivative works, republish, or otherwise use the content and materials in any way for any private or public, for-profit or non-profit purpose, without the express written permission of Mind Yourself Ayurveda.

Privacy

Mind Yourself Ayurveda shall not be responsible or liable in any way for any loss or damage arising from your failure to comply with these requirements.

Indemnification

You agree to indemnify, defend, and hold harmless Esther Seegers, Mind Yourself Ayurveda, from and against any suit, action, liability, or proceeding in connection with any third party claim and/or complaint to which we may be exposed. You agree in this regard to accept exclusive liability for any sums, damages, costs, lawyers’ fees, or court costs for which we may be held liable arising from your violation of these Terms and Conditions.
